To apply for your child’s admission to the nursery, please
download the form, fill it out, and return it to the nursery
by scanning and emailing it to
info@keynursery.com
or by dropping it off at the nursery’s reception.
REQUIRED DOCUMENTS
In order for an application to be considered and a child to
become enrolled at our nursery, the following items must be
submitted to the nursery’s reception:

Completed application form

Completed medical form filled by applicant’s
pediatrician

Applicant’s updated vaccination records
4
Registration fee (to secure your child’s place)
5
5 passport size photos of applicant
6
Copy of applicant’s birth certificate
7
Copy of applicant’s ID card or passport
